Completing your paperwork in advance helps your visit go smoothly and lets our team focus on your care rather than administrative details. You’ll find our New Patient Form, pre- and post-operative instructions for oral surgery and sedation, our HIPAA and privacy acknowledgements, and our cancellation and late policy below, along with Spanish-language versions of our health history and privacy notices. Each form is a downloadable PDF you can print and fill out at home; if you have any trouble opening a form or a question about which ones apply to your visit, please call the office and our staff will be glad to help.
